All patient health care information at Arndt Chiropractic Center is confidential. Therefore, medical records cannot be released to any person without the authorization of the patient or the patient’s legally authorized representative (unless authorized by law).
All requests for the release of medical records must be submitted in writing through the use of a Arndt Chiropractic Center authorization form, and must be dated and signed by the patient or the patient’s legally authorized representative. In the case of a minor, the parent or guardian must sign the authorization.
To submit a written authorization for the release of medical information from Arndt Chiropractic Center, mail the completed authorization to:

Fees have been established for copies of patient medical records/medical information and are subject to change.
No fee is assessed when the information is being sent to another health care provider.

If you are covered by insurance, it is always a good idea to be familiar with your insurance plan and/or contact your insurance carrier before coming to the clinic. How Long Does Insurance Take To Process My Claim?
Insurance can take up to 90 days to process your claim. In many cases, if you paid your out of pocket share at the time of service, no further action is needed. If a claim processes though and is denied or you owe an additional amount, a bill will be sent to you during the next billing cycle.
